Availing of the Italian illness allowance during assignments abroad

Illness allowance in EU, EEA or Switzerland

The Italian state provides protection in the event of illness for both employees, self-employed workers and freelancers.

In the event of illness in an EU or EEA country or Switzerland, the worker is covered by the legislation of the country of the competent institution, i.e., the country in which the worker is insured.

Posted workers may need an A1 certificate to confirm the applicable social security legislation. Additional forms, including S1 where applicable, depend on the worker’s residence/coverage situation and the rules of the competent institution

Illness in a non-EU country

If you fall ill during a stay in a non-EU country, however, the procedure varies depending on whether Italy has signed bilateral agreements with the foreign country.

Where a bilateral agreement applies, the procedure may be simplified and should be checked against the specific agreement and INPS requirements.

– In countries with which Italy has no agreements, sickness benefits are paid only after the duly legalized certificate has been submitted to the Italian authorities. For non-agreement countries, the certificate may need to be legalized and, where required, translated before submission to the Italian authorities.

Submission of the sickness certificate

The worker must send the complete sickness certificate to INPS and the version without diagnosis to the employer within the applicable deadlines and in accordance with the relevant INPS and contractual rules. 

Within the same timeframe, employees must also send their employer the sickness certificate, i.e. the certificate without the diagnosis details.

F.A.Q. about the service

If you fall ill while abroad, on the first day of illness you must contact a doctor in the host country to request certification of your temporary total incapacity to work.
In many EU/EEA/Swiss cases, translation into Italian may not be required under EU social security coordination rules; however, the competent institution may request additional information or clarification.
